Frequently Asked Questions

How do I submit a rebate?

First, please read the entire set of instructions on the back of your rebate slip and be sure to keep copies of all the documentation you are mailing in.

Second, go to www.myalconlensrebates.com to enter your individual rebate code which can be found on the back of your rebate slip and complete the registration process. After registering your profile(s), enter your purchase information and answer a few short survey questions. You will have a chance to confirm your information before you print out the required official rebate form. This form will auto populate with the information you entered when registering.

Note: Please provide your full mailing address including any apartment or suite number necessary for delivery. Also, only ONE rebate submission per envelope AND no P.O. Boxes are accepted, except for residents of North Dakota and where required by law.

Where do I mail my rebate?

If you have completely registered your rebate online, printed your Official Alcon Rebate Form, and included all the required rebate materials listed please mail your Alcon rebate to the address listed below:

One submission per envelope. If you are submitting for multiple rebates, please mail each in a separate envelope.

After registering my rebate online, I was not able to print my rebate form. How do I print my rebate form?

If you completely registered your rebate online, your will be able to reprint your Official Alcon Rebate Form by logging into your profile and selecting “Track Rebate” from your profile homepage. Once you have selected the specific rebate, you can choose to reprint the form by selecting "print." Please allow up to 24 hours for your rebate to be displayed in the tracking results. Additionally, please allow 6-8 weeks for rebate processing; this period will not begin until your supporting documentation has been received at the address listed on your official rebate form.

What if I have multiple rebates to submit?

Once you have created a profile online at www.myalconlensrebates.com you can create multiple patient profiles within your household. Each rebate will be registered separately by following the instructions for “How do I submit a rebate?”. If you have multiple rebate forms, please submit each rebate in a separate envelope.

Do the rebate offers expire?

Yes, rebates typically are special offers with limited timeframes. Please be sure to check the terms and conditions of the offer on the back of the rebate slip and submit the required documentation before the deadline.

What is the UPC?

The UPC is a bar code (a series of vertical black lines with numbers under them). Usually, it includes a 12-digit number that begins with a zero or a seven. These 12 digits include 10 digits directly below the barcode plus one digit to the left and one digit to the right. The UPC is located on the product packaging. Please send in one original UPC for each brand of product purchased and make a copy of the UPC just in case the mail is lost and you need to resubmit it.

What is the difference between purchase date and postmark date?

Rebate offers are planned for very specific time frames. The purchase date is the day on which your product purchase is made. This day must fall within the promotion time frame specified on the rebate slip you received. The postmark date is the day on which the post office stamps your envelope. You will not qualify for a rebate if you do not mail your required supporting documents by the specified postmark date on your rebate slip.

Why do you need purchase date and eye exam/lens fitting date?

For most offers, product purchases must be made within 90 days of the eye exam (or contact lens fitting) to be eligible for this rebate. See the terms and conditions listed on the back of your rebate slip for more details.

I lost my fitting or eye exam receipt, how do I get my rebate?

Please go back to the eye care professional where you had your fitting or eye exam and ask for a reprinted receipt.

Can I send a copy of my fitting or eye exam receipt and hold onto the original?

Please review the terms and conditions on the back of your rebate slip carefully. This rebate slip will specifically state whether or not the original rebate receipt must be sent.

What information do I need to send in to get my rebate payment?

The required information can vary for each offer. Please refer to the back of the rebate slip you received from your eye care provider to find the required supporting documents. Be sure to keep copies of all the documentation you are mailing in.

Can I submit my rebate completely online?

You cannot submit your rebate request completely online at www.myalconlensrebates.com. Online, you will be asked to register your rebate by creating a profile and entering specific purchase information. Then, you must print out an official rebate form (from the online site) that captures all of the information you entered online and mail it in with the required supporting documentation listed on your rebate slip.

What if I don't have a printer or experiencing difficulties printing? How do I submit?

You are able to reprint your Official Alcon Rebate Form by logging into your profile at a later time and select “Track A Rebate” from your profile homepage. Once you have selected the specific rebate you can choose to reprint the form.  6-8 weeks is allowed for rebate processing and this will not begin until your supporting documentation has been received at the address listed on your official rebate form.

How do I track the status of my rebate?

You can track your rebate by logging into your profile at www.myalconlensrebates.com and selecting “Track A Rebate” from your profile’s homepage.

How long will it take for my rebate to be processed?

Please allow 2 weeks for the postal service to deliver your rebate submission to the address listed on the official rebate form. A total of 6-8 weeks is allowed for rebate processing and this will not begin until your supporting documentation has been received at the address listed on your official rebate form.

What if something is missing from my submission? Will I be notified?

If you have not included some required information or documentation, your submission will be invalid. You can Track the status of your rebate by logging into your profile at www.myalconlensrebates.com and selecting “Track A Rebate from your profile’s homepage.

What form will the rebate payment be sent in?

Please refer to the terms and conditions on the back side of your rebate slip for more details on the form of rebate payment you will be sent with a valid rebate submission.

Who should I select as the Cardholder? Whose name will be printed on the rebate payment?

The Cardholder is the first and last name that will be printed on the rebate payment for a valid submission.

A single household account can have multiple patient profiles. When submitting a rebate online from your household account, you will be prompted to select the “Cardholder Name” which is the patient profile whose name you would like to be printed on the rebate payment. Then, you will be prompted to select the “Patient Name” which is the patient profile who is the contact wearer for this particular rebate.

Will the personal information I enter on the site be given to other companies?

No, as stated in our Privacy Policy, ALCON® does not share your information with other companies. If you opt in to receive ALCON® information, you may receive product-related messages from ALCON.
